Medical documents required at the
beginning of the school year
The Avenor medical team is ready to treat the health of our students with the utmost care and responsibility.
We rely on your support and cooperation to ensure that together we can keep the community safe.
On the first day of entering the community, the children will bring the self-declaration filled in by the parents.
The rest of the documents can be brought in the first week of school to the medical office. Or sent to the following emails:
- For school: cabinet.medical@avenor.ro
- For nursery: medical.office@avenor.ro
List of compulsory medical documents for the school year 2021-2022:
Self-declaration (brought on the first day of school)
Only for students in School (not for the children in the Nursery)
Certificate issued by the family doctor with the specification “suitable for taking physical education and sports classes” (brought in the first week of school.)
FOR NEW STUDENTS ONLY:
Self-declaration (brought on the first day of school)

The new students will bring in addition, in the first week of school, to the medical office:
Medical certificate for enrollment in the community
Vaccination form
The child’s medical history, completed by the parents
According to Order no. 1002/5057/2015 issued by the Ministry of Health, the certificates are valid only if they are filled in on the standard forms that you can find in the links above.
