Medical documents required at the
beginning of the school year
Important medical information for NEW STUDENTS
The Avenor Medical team is prepared to treat the health of our students with the utmost care and responsibility. However, we also rely on your support and cooperation to ensure that together we can keep the community safe.
- An Epidemiological certificate for joining the community issued by the paediatrician (“Aviz epidemiologic pentru intrarea în colectivitate”) has to be brought physically, on the first day of school. Please be aware that the issue date of the certificate must be 48 hours prior to attending nursery.
- Please COMPLETE and UPLOAD the below medical documents to your personal account on Open Apply, by Monday, August 23rd. We cannot accept documents in physical form. The list of required medical documents can also be found in the Open Apply account, in the Checklist section.
*Medical documents are not required for students who were part of the Avenor community in the previous school year.

Mandatory medical documents for NEW STUDENTS
Medical certificate for enrollment in the community (issued by the paediatrician/family doctor)
Vaccination form (issued by the paediatrician/family doctor)
Medical Record (the child’s medical history, filled in by parents)
According to Order no. 1002/5057/2015 issued by the Ministry of Health, the certificates are valid only if they are filled in on the standard forms that you can find in the links above.
